Daniel Summers
@danielrsummers.bsky.socialH5N1 update from Dr. Bonnie Henry:
•Teenager with H5N1 infection remains in critical condition (stable)
•34 HC workers had exposure. All have been through 10-day incubation period, monitored/tested. All negative
•16 close friends/family had exposure, offered prophylaxis, ongoing follow-up
•No additional cases identified at this point
•Tests conducted on multiple species of animals, including potential source (pet dog). Thoroughly investigated, all addtitional tests in dog negative
•Was there common exposure at school? No common exposure ID'd. No one w/ symptoms at school
•Source at this point is unknown
•Teenager was infected with Clade B 2.3.4.4.b, genotype D1.1 (same identified in poultry outbreaks in BC)
•Not related to genotype identified in ongoing US dairy outbreak
•Closest (not exact) genomic match to wild geese in Fraser Valley
•Mutation(s) identified in child infection (I believe referring to at least PB2 E627K, and other changes RE to sialic acid binding) "may" indicate progressive human adaptation
•No evidence of any human-to-human transmission associated with this case
•No evidence of H5N1 infected dairy herds in Canada
•Environmental exposure to places with infected birds (waterways, lakes, ponds) via bird droppings is a risk
•Missed this earlier (important), but they indicated they only tested those exposed who were symptomatic (family/friends/school/HC workers)
